Summer Camp Registration Form

Camps will begin at 9am and run until 5pm. Please provide a lunch for your child as well as two snacks, one for morning and one for afternoon time. Please also provide a refillable water bottle for them to have throughout the day. Campers will need to be signed in in the morning and signed out at pick-up time. Parents and custodians who will be picking up and dropping off must be listed with contact number.

Please submit payment for each camp you have selected by calling the Box Office at (904) 375-9547. REGISTRATION WILL NOT BE COMPLETE WITHOUT RECEIPT OF PAYMENT. All Major credit cards, checks and cash are accepted for payment. Payment covers camper’s tuition, camp t-shirt, and two tickets to the end-of-week showcase. Additional tickets for the showcase can be purchased through the Box Office or Online on our Summer Camps Page.
